CONFIRMATIONS- An automatic confirmation will open when you hit "Submit" on the form below. If there are any errors or changes in your submitted form, please complete a new form. The most recently submitted form will be used.
This UAD Event will count towards UKC Dock Jumping Titles.
For dogs to get UKC dockjumping titles, they will need be registered with the UKC. For more info on the different kinds of registration: http://www.ukcdogs.com/Registration.htm
Registration with the UKC gives you access to all their performance events: obedience, agility, hunt tests, etc.!
Participations in the UAD-UKC Title Program is not mandatory. It is not necessary for your dog to be UKC registered in order to pre-register or participate in a UAD Event.
ALL dogs will still earn annual UAD Titles, at all UAD Events (not Demos),
which are different than UKC Titles, regardless of UKC Title status.
UKC Titles count for the lifetime of the dog and are recorded on the dog's registration.
UAD Titles are given annually.

IMPORTANT INFORMATION:
2008 FEES- $15 for the first dog, $10 for second dog, $5 for any remaining dogs in a family, per activity, unless otherwises indicated. Please, pay at the event in cash or check. We do not take credit cards. We do not take payments before the event, in order to save spots. Your spot will be saved just be pre-registering.
EVENT PROCEDURES- Everyone MUST sign a Liability Waiver ONCE per event! Everyone MUST check in with the Secretary during the scheduled REGISTRATION time for each activity, even if you are Pre-registered. Otherwise, you will not be included in the running order. Additions will not be made after the activity has begun.
